Grounded Performance Group Training Sessions and Personal Training Release, Covenant Not to Sue, and Waiver

I hereby acknowledge and agree to the following:
Grounded Performance coaches are not physicians and are not trained to provide medical diagnosis, treatment, or advice. If I am unsure about my ability to perform intense exercise, I agree to consult a physician immediately.

I understand that if I feel tired, pain, or discomfort during any session, I should stop and consult a physician if necessary.

Grounded Performance training carries risks, including injury or illness such as musculoskeletal injury, cardiovascular issues, or even death, including exposure to contagious diseases like COVID-19. I assume full responsibility for the risks of participating in these activities, including natural elements and illnesses.

I consent to participating in Grounded Performance’s programs, which may include strength, endurance, and cardiovascular exercises. I understand that exercise can carry risks such as muscle injuries, abnormal blood pressure, and, in rare cases, heart attack or death. Every effort will be made to minimize these risks.

All personal information regarding my fitness level and progress is confidential and will only be shared with my physician with my written consent. I understand that I can discontinue participation at any time.

I agree to release and waive any responsibility from Grounded Performance and its affiliates for any injury, illness, or property damage related to my participation in these programs.I acknowledge that photos and videos may be taken during classes and events, and I grant permission for my likeness to be used for marketing purposes.

Personal TrainingRates: Rates will be agreed upon by the trainer and client before the start of the session.Attire: Wear appropriate workout attire and dress in layers that breathe, as summer heat can affect performance. A water bottle is required to stay hydrated. No flip-flops, wear comfortable athletic shoes or no shoes if you prefer.

Training Sessions: Sessions will begin promptly at the scheduled time. Payment is required before participating, and your registration is not guaranteed until payment is received. Sessions last 60 minutes unless otherwise agreed upon by the trainer.

Cancellations: A 24-hour notice is required to cancel or reschedule a session to receive credit. Failure to cancel within this time or no-shows will result in being charged for the full session. Exceptions may be made for medical emergencies at the trainer’s discretion.

Weather-Related Cancellations: In the event of severe weather, Grounded Performance may cancel classes or training sessions for safety reasons. Members will receive credit for any session canceled due to weather conditions.

Reimbursement or Credits: Grounded Performance does not offer refunds or credits. If an injury prevents you from completing the sessions, a partial refund may be issued with a physician’s note.

Injury: You are fully responsible for any injuries sustained before, during, or after training sessions. Grounded Performance and its trainers are not liable for any injuries. Clients must remain focused during training and communicate any discomfort.

Effective Date: 1/24/2025
